Public health systems must simultaneously expand access to essential services, protect populations from health threats, reduce inequities, and remain capable of learning and adapting under conditions of fiscal, demographic, epidemiological, and technological change. This qualitative literature review examines how governance, financing, primary health care, health workforce capacity, digital information systems, community participation, social determinants, and public health functions interact to shape system performance, with particular attention to Indonesia. Using a narrative-integrative approach, peer-reviewed literature published primarily from 2020 onward was purposively identified and synthesised thematically around system architecture, universal health coverage, policy implementation, equity, digital transformation, and resilience. The review finds that effective public health performance emerges from coordinated system relationships rather than isolated investments. Universal health coverage is strengthened when financial protection is matched by service availability, quality, workforce distribution, referral capacity, and accountability. Digitalisation can improve surveillance, coordination, and learning, but requires interoperability, governance, privacy protection, and institutional capability. Community participation and intersectoral action are essential for addressing social determinants and improving policy legitimacy
