Abstract

Diabetic peripheral neuropathy (DPN) is a prevalent and disabling complication of diabetes characterized by sensory loss, neuropathic pain, impaired balance, foot ulceration risk, and progressive deterioration of peripheral nerve function. Its pathogenesis extends beyond hyperglycemia and involves oxidative stress, mitochondrial dysfunction, advanced glycation, neuroinflammation, microvascular impairment, disrupted neurotrophic signaling, Schwann-cell dysfunction, and progressive axonal degeneration. Because currently available pharmacological therapies predominantly target neuropathic pain rather than the underlying neural injury, interest has grown in compounds capable of modifying several pathogenic pathways simultaneously. Palm-derived tocotrienols are of particular interest because clinical studies have demonstrated measurable improvements in selected sensory and motor nerve-conduction parameters and increases in circulating nerve growth factor. However, electrophysiological improvement has not consistently translated into comparable symptomatic relief, while some gains diminish following treatment withdrawal. This critical qualitative review synthesizes peer-reviewed evidence published from January 2020 through December 2024 to explain this apparent discordance. The review argues that nerve-conduction velocity, small-fiber integrity, neuropathic pain, and functional recovery represent overlapping but non-equivalent domains of DPN. Current evidence supports biological and electrophysiological activity of tocotrienol-rich vitamin E but remains insufficient to demonstrate durable structural neuroregeneration or definitive disease modification. Future trials should prioritize early-stage DPN, patient stratification, pharmacokinetic characterization, multidomain neurological endpoints, and post-treatment follow-up
